The staff member accompanied the children to the bathroom. When they were done, one child did not want to leave and she pulled on his arm accidentally to lead him out of the bathroom . It did result in a Nursemaid’s elbow injury which was confirmed by the physician he saw after childcare, and doctor said this type of dislocation is commonly referred to as a "Nursemaid's Elbow" which is a common injury in children when their arm is pulled or lifted forcefully".

The outdoor play yard was observed with no latching mechanism at all on one gate, in the preschool yard, stating it fell off. The door does not shut but the provider put an elastic band on gate door and fence to keep it from swinging opened.
