During the time of inspection, licensor observed napping equipment partially blocking the exit door in several classrooms. Director has addressed this with staff supervising in each room for correction while licensor was present.
All staff memebers listed in MERIT have have either expired Blood Borne Pathogen training or have not completed it. All staff will need to take or renew their Blood Borne Pathogen training and update in MERIT. - CPR/First aid for several staff members have expired. All staff have to be updated with this training, uploaded in MERIT and have training cerificate on file for licensing review. - All staff memebers listed in MERIT have have either expired Safe Sleep training or have not completed it. All staff will need to take or renew Safe Sleep training and update in MERIT.
There are several areas of climbing equipment that does not have sufficent ground cover or any ground cover: - Little Tikes climbing structure is placed directly on the ground without, hard surface - Metal tunnel with bars, metal helicopter (not sturdy), and little tikes minature climbing structure, does not have any ground cover. - Main play structure with swings and slide will need to have ground cover (wood chips) added to cover hard surface.
In the outdoor licensed space, there is a wheel/tire anchored in the ground that has a piece of broken metal with sharp edges that can caused severe cuts. - In the school-age room, ther are two furniture peices used in the dramatic play/kitchen area, the sink and pantry shelf has splintered corner and loose piece, will need to be replaced or removed. - Water temperature for the handwashing sink in the Waddler classroom was taken by licensing staff, temp mearsured at 130.2 degrees. Temp must be 120 degrees or less
Heater barrier in the school-age room is partially opened leaving heater accessible to children - Candle that was previously burned, will need to be removed
During the time of inspection, licensor observed dinner rolls with mold in the kitchen. Rolls removed while licensor was onsite.
Several child files observed are missing immunization records, child specified with staff present to keep childs name confidential.
During the time of inspection, licensor observed a toliet plunger in several classrooms, accessible to children. - Bleach/disinfectant solutions found in child accessible locations in several classrooms. Will need to placed in location that children of that age group cannot reach.
Several child files missing date entered, one missing dental information. Child specified to director to keep child information confidential.
Cord in the Toddler classroom, near the window has a loose cord, connected to power surge, will need to made secure to prevent from making a loop. Outdoor equipment has a rope with wood steps that are partially broken apart, has splintered edges. Shelved in the hallway of school-age walkway and shelves in hallway of toddler and preschool hallway need to be secured or removed Outlet next next to the sink in the school-age bathroom is not a GFCI outlet Power strip with surge protector is accessible to children in the Toddler classroom next to the window.
Cleaning and santizing products found accessible Infant classroom, cabinet below sink has pink cleaning solution that says "keep out of reach of children" Preschool classroom, bleach bottle near sink School-age classroom rubbing alcohol in large cabinet, bingo stamps labeled "keep out of reach of children", lysol spray in the large cabinet. Storage Room in hallway unlocked Janitorial supplies accessible to children including a plunger and a broom with the bristles against the floor, and vaccums in bathrooms
