Extension cord was observed in licensed play area near one of the cubbies that was accessible to children. Bathroom heater observed to be rusty and flaking off. Some bathroom tiles were observed to be broken. Window seen from outdoor space was observed to have flaking paint around the window seals and frame. This window is accesible to children as it not far from the ground.

Emergency exit door in kitchen space was observed to be blocked off by boxes. Emergency exit doors (in kitchen and sleeping area) are also blocked off by a baby gate that requires special knowldge to open or access from the play area. Both gates are observed to be bolted into the walls.
