Based upon information gathered from interviews with child care center staff and enrollment records reviewed, there is not sufficient evidence to suggest that the person the child was released to was authorized by that child's parent to have unsupervised access to the child. Determination of this finding was as a result of staffing between this licensor's supervisor and area administrator.
Upon interviewing administrative staff, the information shared with the licensor confirms that a dangerous incident occurred within close proximity to the child care center in which emergency services were required, and that this incident was not reported to the department within twenty-four hours.
Based on the information gathered from the inspection, it was determined that the early learning provider did not submit a written incident report to the department within 24 hours when a serious injury in a child happened at the center. Based on the information gathered from the inspection, it was determined that there was insufficient evidence to suggest the early learning provider reported the incident by telephone within the required timeline to the listed individuals, department and other government agencies when the incident occured.
A beading cord with beads was observed to be hung on a cabinet door handle in the kindergarten prep classroom that was accessible to children and posed a strangulation hazard at the time of inspection (The violation was corrected on site).
MERIT reflected that 5 staff members did not complete the child care basics training at the time of inspection. MERIT reflected that a lead teacher's CPR and first-aid were expired (2/10/2024) at the time of inspection. MERIT reflected that three lead teachers' safe sleep were expired at the time of inspection.
At the time of the onsite Monitor Visit, there were blinds in classrooms #230 and #233 that had blinds with a metal chain pull that formed a loop and was accessible to children in the classroom. At the time of the onsite Monitor Visit, there was a fishbowl (Round 16" diameter), sitting on a 13" high wood box, that was positioned above a normal tamper resistant outlet and accessible to children. At the time of the onsite Monitor Visit, there was a fishbowl (Round 16" diameter), sitting on a 13" high wood box, that had an electrical pump plugged into an outlet less than a foot away and accessible to children in the class. At the time of the onsite Monitor Visit, multiple staff that had been hired for more than three months did not have childcare basics completed in MERIT.
