The laundry room is accessible to children as there is not a door or gate preventing access. A child was observed to enter the laundry room alone. Inside the laundry room is a dryer that is not secured. The placement of the washer and utility sink create an opening which poses an entrapment risk. Another circular opening in the wood at the utility sink creates another entrapment hazard as it measures more than 3.5 inches. Completed Date Disputed Date Disputed
