0186(1) - The individual care plan was not signed by the doctor and did not include the required information listed in section a and b of the WAC.
0182(2) - Medication for one child with a known allergy, as listed in the medication authorization plan, was not obtained by the program from the parent.
It was observed that the "Wal-Dryl" non-prescription medication suitable for children 6 years and older was missing a prescription from a medical professional, including directions for administering the medication and dosage amounts for a child of 5 years old, as directed on the package.
A shaving foam and sanitizer were observed located in the classrooms in a place accessible to children. Both have a statement printed on the package: "Keep out of reach of children."
It was observed that the CCB was not recorded in Merit for one teacher. The director did not have a certificate to prove the class completion.
