0260(2)(a)(b) - Licensor observed storage room door to be unlocked and accessible in the (T) classroom.
0106(3) - Multiple teachers who have been employed longer than 3 months have not completed the childcare basics training.
0165(2)(d) - Provider stores nap mats in plastic/zippered linen bags. At the time of the visit, the bags were stored (unzipped) under the children's nap mats for nap time. Leaving these bags accessbile during nap, next to the child, poses a suffocation hazard. Provider needs to come up with another storage plan during nap time.
0165(3)(f) - Metal shelf in room 109-Venus that is located in an alcove in the corner does not have any "lips" to prevent heavy items from falling. The shelf has many items stacked haphazardly that could easily fall off the shelf. Provider needs to reorganize this shelf in a way that prevents items from easily falling off.
0260(1)(b) - cleaning supplies stored on shelf under food prep table need to be stored in an alternate space.
