At the time of this inspection, licensor observed 6/6 child files reviewed did not have current and/or complete immunization records - Report dates on forms were observed to be from 11/28/2023 for children A.A, Z.A., and R.A., and 11/24/2023 for children I.A. and L.A.. CIS form for child I.N. was marked "Not Complete" with vaccinations listed overdue including Polio due 9/22/2025 & Varicella due 1/17/2026.
