At the time of this inspection, licensor observed 1 tricycle with visible rust accessible to children and 1 metal toy truck with flaking paint and visible rust on the playground. Provider corrected onsite by removing the tricycle and toy truck outside of the licensed space in an area inaccessible to children.

At the time of the onsite monitor visit, there were four shades with metal chain pull cords that were loose and formed a loop large enough to fit an adult head in and was accessible to children present. At the time of the onsite monitor visit, there were multiple cords and wires on classroom counter top that were loose, capable of forming a loop around a child's neck, and accessible to children present. At the time of the onsite monitor visit, there were two screws sticking out of the front of the playground pirate ship and accessible to children.

At the time of the onsite monitor visit, there were childproof door handle devices installed on the inside of all classroom doors in rooms 123 and 125. Both doors (main and door to playground) marked as exits.
