Clean and healthy environment

Walls around the home were observed to have chpping paint, as well as the framing all around the home. Chips and cracks were observed. The wall near the door was also observed to be broken with a rectangle hole of about one foot. This hole is exposing wood and is not a cleanable surface. An air freshner was observed in living room space near the door area while children were present.

Emergency gate is not self-latching. Provider must tighten spring in order for door to close and latch properly.

Cords and wires were observed accessible to children in licensed living room space. The following cords were located in living room next to bookshelf, (phone charger), on cubby under tv which had two drawers that had tablets with loose cables and cable coming down from tv. Carpet inside licensed was observed to be lifting creating a tripping hazard. Cubby under tv space was observed unstable, creating a tipping hazard. Outlet in hallway space on kitchen wall is not tamper-resistant and accessible to children.
