During an onsite visit on 6/5/2025 an infant was observed sleeping in bouncer seat/chair on two separate occasions. During the first incidence the child was asleep in the seat for more than 10 minutes. On the second occasion the licensor and regional director had to ask the staff to move the child to a crib.

During multiple onsite visits, the back building door leading to the playground was propped open, or unlatched. Door leads to playground. During onsite visits it was observed it was observed when the backdoor was open, playground gates to dumpster area and/or parking lot were unlatched and open, also. Additionally, noise mechanism on emergency exit doors were not working or in use at the time of visits.

During multiple onsite visits, a noise mechanism was not observed on classroom emergency exit doors. The center back door emergency exit was propped open; leading to potential hazards including: access construction materials, standing water, the dumpster and parking lot.

During onsite inspection on 2/26/25, prescription medication was observed in the toddler classroom, on the corner of the counter, in reach of children.

During multiple onsite complaint inspections, the back emergency door exit was open/unlatched leading to a pathway to the unsecured storage/ dumpster area. The dumpster storage area fence was unlatched and opened. The adjoining parking lot gate was unlocked. The laundry room storage area door was unlocked.

Not all food stored appropriately, food was observed not in origin packaging and was not dated and labeled. Infant food observed to be expired. Staff removed expired food from the center and immediately.

Under sink area in the toddler rooms observed to be accessible to children. Under sink contained hazards, such as plastic bags. Cords observed to be accessible in some areas of the building, multiple cords observed in the schoolage. Outdoor fencing observed to be separated from the ground exposing sharp edges. A piece of siding on the schoolage/preschool playground observed to have sharp jagged edge. Cabinet in schoolage room observed to need repair, including a broken door not on the hinges. Center began corrections of items immediately following visit.

Cleaning chemicals, along with various items (sunscreen, teacher materials, etc.) stored on the counter with food, water cups, and various other food prep supplies throughout the building. Cleaning chemicals and items labeled "Keep outreach out children" observed to be accessible in Preschool 4 and Schoolage bathroom closets. Toilet brushes and plungers observed in some classroom. Cleaning items such as brooms, observed to be accessible to children. Some classrooms have wall securing mechanisms, but they were not observed to be in use. Center staff immediately began making corrections.

Did not see initial training of Safe Sleep for J.A. Expired annual Safe Sleep training in MERIT shows for S.K., M.D., M.O., and M.S. Expired Food Handler's and Safe Sleep training for staff who was working before but recently paused working, H.M. Will do before returning to work per Director. Expired Food Handler's in MERIT for R.M., M.H. Many staff have not updated annual BBP training. Expired FA/CPR dates for M.Q., M.H. and M.S.

Toilet brushes, dustpans, brooms were accessible to children in bathroom areas and classroom. Closets in the bathroom areas did not have locks or another way to make it inaccessible to children.

The teacher was not adequately anticipating the need for active supervision during the time the schoolage children were playing a game involving physical contact or wrestling. Center director reports video footage shows during the incident the teacher appears she is on a phone or technology device, therefor not able to engage in active supervision. The teacher should have anticipated an incident or injury due to the age of the children and the nature of game, involving physical contact.

Equipment and surfaces in outdoor early learning space

The 3-5 yo climber states clearly it is intended for use by 3-5 year old children. The school age children are climbing on it simultaneously and using the climber which is not manufactered for this age group. There is a short slide/climber on the toddler play yard that did not have a 6 ft fall zone. The jeep climber and larger 3-5 yo climber did not have a 6 ft fall zone. The other large 3-5 yo climber was too close to a boat and did not have a 6 ft fall zone.

Supervising children

The teacher on the play yard appeared to be talking on the phone and standing several feet away, looking at the multiple school agers standing and playing on the 3-5 yo climber. This does not appear to be active supervision.
