Requirements of 19 CSR 30-60.080 Fire Safety were not met as evidenced by fire drills were not held at least one time every three months.

Required correction: Fire and tornado drills shall be completed and recorded as required.

Required correction: Hazardous material(s) shall be inaccessible to children and stored to prevent cross-contamination of food and food-related items.

The facility did not meet requirements for child enrollment and health information on file as evidenced by three child files did not have the full name of the parent(s), guardian or legal custodian, home address, employers’ name and address, work schedule and home and work telephone numbers.

Required correction: An individual file with required information for each child shall be maintained.

Doors to the bathroom or stalls did not meet requirements as evidenced by there was a lock on the bathroom stall.

Required correction: Doors to bathrooms or stalls, used by children below the first grade, shall not have locks or latches.

Requirements for the outdoor play area were not met as evidenced by the fence had openings that were greater than 3 1/2 inches and allow entrapment - the openings measured gap in gate closure of 4.5 inches.

Required correction: The outdoor play area shall be on or adjoining the property and fenced as required.
