Hazardous items as follows were accessible to children: diaper cream. The item(s) was/were located in cubby in the 2 year old room.
Required correction: Hazardous items shall be inaccessible to children.

The facility's outdoor space was not safe for children's activities as evidenced by there were weeds by the slides and along the side of the fence.
Required correction: The outdoor play area shall be clean, safe and well-maintained as required.
