A child was left without competent supervision as evidenced by at 8:55 AM while parking I observed the children on the playground going inside. As I got out of my car and walked to the building I witnessed Child A, four years old, standing outside the closed classroom door on the playground. I paused and looked around the playground and did not see any adult outside. When I reached the gate, I asked Child A if they were alone on the playground. Her response was yes. I quickly surveyed the playground again and saw no adults. I told her to come to me. I opened the gate and took her hand. After I took her hand, a teacher M.T.P. came up from behind me and said I will take her. I am a teacher here. The three of us walked to the front door together. Patricia Palacios opened the door for us and I informed her that I had found the child outside alone. I could tell by her reaction that she was unaware the child had been unsupervised outside. Patricia walked into the classrooms trying to find out what happened. The teachers were unaware that the child had been left outside.

Required correction: The facility shall provide competent supervision for each child as required.
