The walls located on the wall by the door going outside to the playground was/were not finished with material which can be cleaned easily as evidenced by the wall was in the process of being repaired due to a new door being installed.

Required correction: Walls, ceilings, and floors shall be made of approved materials, easily cleaned and in good condition.

The facility did not have a fenced outdoor play area that was on or adjoining the day care property and was accessible without hazard as evidenced by the fence had openings that were between 3.5 and 9 inches and allow entrapment - the openings measured approximately 6 - 7 inches.

Required correction: The outdoor play area shall be accessible without hazard and provide a safe fence around the outdoor play area as required.
