Requirements for indoor play equipment and materials were not met. Toys/materials were not in good condition as evidenced by gym mats under indoor equipment in gym were worn and had exposed foam which is not able to be sanitized.

Requirements for fire safety inspection were not met as evidenced by the inspection has not been conducted.

The facility's outdoor space was not safe for children's activities as evidenced by the presence of exposed landscaping material which poses a tripping hazard.

Required correction: The facility is hereby placed on 20-day notice that a corrective action plan must be developed to ensure compliance with the above statute/rule violations. A copy of the corrective action plan must be received by the Office of Childhood within 20 days of this letter.
