The walls located in the infant/toddler unit was/were not in good condition as evidenced by a half door between the two classroom spaces had duct tape covering damaged wood. Part o the duct tape was hanging free and had splintered wood hanging from it. Parts of the splintered wood was small enough to be swallowed by the infants/toddlers in that class. Three children under 1 year of age were present in the classroom. Director S.B. attempted to secure the duct tape but it would not hold to the door.

Required correction: Walls, ceilings, and floors shall be made of approved materials, easily cleaned and in good condition.

The provider did not follow required procedures for accidents, injuries and emergency medical care as evidenced by parents were not notified immediately when Child A fell on the playground and stated he hurt his shoulder and was crying. On February 29, 2024, CI Greer interviewed Parent A, Child E, Caregiver K.B., and Caregiver M.H. Parent A said Child A had tears in his eyes and said he was racing and tripped and fell on the playground when she picked him up on February 27, 2024. Child A said his shoulder and arm were hurting. Parent A took Child A to the hospital where it was determined that he had a broken collarbone. Child E said she remembers Child A falling on the playground. Caregiver K.B. said she remembers Child A falling on the playground on February 27, 2024. Caregiver Haniyeah said on February 27, 2024, Child A was running and fell down on the playground. She said he was crying and grabbing at his shoulder and saying he hurt his shoulder. She did not notify Parent A of the accident/injury. On March 5, 2024, CI Greer interviewed Child A and Child C. Child A said he fell down and pointed at his upper arm when asked where he was hurt. Child C said she saw Child A racing with other kids and fall.

Required correction: The provider shall follow procedures as required in the case of a child's accident, injury, or emergency care.

A caregiver was not knowledgeable of the needs of children as evidenced by during an inspection interview Malaika Firdous was unaware of the ages, history or developmental needs of the children in her care. When asked Malaika Firdous did not know the names or ages of all the children in her care. CI observed a redness to the child's left hand and pointed it out to Malaika. Malaika did not know how that happened and went to touch the child's hand and the child pulled it away from the caregiver.

Required correction: Caregivers shall meet child care personnel requirements.

A caregiver exhibited behavior that did not show the individual to be of good character and intent and qualified to provide care to children as evidenced by Malaika stated that it was her first day. During the encounter the director had to tell the staff member the name of the child she was holding and that the child was fearful of strangers. When the CI interviewed the director asking why the staff member did not know the names and ages of the children in her care, the director stated Malaika was new. CI observed personnel file for Malaika and the new hire orientation stated the start date as May 30th, 2023. CI asked the director if Malaika had been trained, the director responded and said they don't do that kind of training but they do the state training online. The director had provided a completed and signed new hire orientation for Malaika (signed by Director and Malaika). Later the director stated again they do not have that kind of training but assured CI that Malaika had taken safe sleep and health and safety training. Another staff member Howah Alhassan interjected saying that Malaika did receive in person facility training by her for approximately 9 days in May. Then the director agreed with Howah, that Malaika did receive training.

Required correction: Caregivers shall meet child care personnel requirements.

An individual was present on the premises that presented a threat to the health, safety or welfare of the children as evidenced by Malaika Firdous was heard using a raised voice saying, "why are you like this?" as the CI was opening the door to the infant room. Malaika was then observed holding a 1 year old child above her head with her hands around their rib cage and the child facing her. While holding the child in the air child Malaika was shaking the child. The CI observed the child's head moving back and forth and the child did not appear to be in control on their head. The child was observed to be upset with a flush face and making sounds like crying but with no visible tears.

Required correction: No person who presents a threat to children shall be on the premises during child care hours.

When the CI spoke with the director regarding the incident the director S.B. stated that this is how our culture does things. Souad also stated that they had no concerns with that staff member.
