In the outdoor play area a broken wooden garden bed with splintering edges and several exposed nails, and the fiberglasss edge of larger sandbox with splintering/broken edges and cracks, accessible to children and posing potential safety hazards. Observed 3 bolts with more than 2 threads exposed on both gates on the playground, accessible to children and posing potential protrusion hazards.
Required correction: Immediately ensure indoor and outdoor equipment, materials and furnishings are sturdy, safe and free of hazards. Document correction in written response. Correct by 01/08/2024
