In the Cardinals classroom, observed a container with broken crayons. The broken crayons were small enough to fit in a choke tube posing a potential choking hazard.
Required correction: Toys and materials accessible to children under three years of age must not be a choke hazard. Corrected at time of visit-Staff took broken crayons and replaced them with large crayons that would not fit inside of a choke tube on 11/20/25. No response required. Correct by 11/20/2025

In the Cardinals classroom, observed a surge protector with uncovered outlets.
Required correction: In rooms used by children, all electrical outlets that are accessible to children must have protective covers, or safety outlets must be installed. Corrected at time of visit, staff pulled out surge protector and the outlets that it was pulled out of were safety outlets. No response required. Correct by 11/20/2025

IPS was observed to be doing breaks during walk-through. Reviewed IPS's schedule and observed her to not be scheduled to work at least 60% of operating hours (only 4 of 11 hours of operation for the Infant classroom) on 2/1/24.
Required correction: In the infant program, there must be a qualified infant program supervisor present at least 60% of each day that the program is in operation. Correct immediately and respond with verification of compliance by 2/8/24. Correct by 02/01/2024

In classroom Ducklings (Todd B) on the changing table, observed 1 package of Kirkland brand wipes lableled "Keep out of reach out children" to be accessible to children, posing a potential safety hazard.
Required correction: All items labeled keep out of reach must be inaccessible to children at all times. Corrected at time of visit-staff stored items inaccessible to children on 2/1/24. No response required. Correct by 02/01/2024
In classroom Robbins (Pre K B), observed a glass mirror on the windowsill with children under the age of 5 years in care.
