Identified on APP- 400833 that provider requested capacity of 167 and paid $801. Identified through provider statement that they would like to license facility for 171 children. Identified that the fee should be $813 and an additional $12 must be paid to the Department.

Facility to be missing the policy regarding identifying recalled toys and developing a plan to remove those items.

Required correction: The procedure must be included in the policies. CORRECTED AT THE TIME OF THE INSPECTION. Program Manager included the porcedure in parent handbook. No response required. Correct by 07/04/2026

Identified through Toddler staff statement and management statement that the Toddler classrooms walk through the Preschool Playground to get to the Toddler playground.

Required correction: The Toddler programs must be completely separate from the Preschool program. Correct prior to approval of the license and submit documentation with a written statement of compliance to specialist. Correct by 07/04/2026
