At the time of visit, a child left a classroom and went to another classroom without the direct supervision of the teacher. Confirmed through staff statements, the child is allowed to leave the room alone to go to another room for the afternoon class session showing a lack of knowledgable decision-making, judgment, and concern for the proper care and well-being of the child.
Required correction: Immediately, all staff must demonstrate knowledgeable decision-making, judgment and concern for the proper care and well-being of children at all times. Submit a written statement of compliance by 12/12/2024. Correct by 12/05/2024

At the time of visit, a staff purse in the Enrichment classroom was hanging on a low hook and accessible to children.
Required correction: Immediately, all staff purses and other hazardous items must be inaccessible to children at all times. Corrected at time of visit, the teacher put the purse on a high shelf making it inaccessible. Submit a written statement of compliance by 12/12/2024. Correct by 12/05/2024

At the time of visit, in the Enrichment classroom, a power strip under the teacher desk had three exposed electrical outlets and was accessible to children.
Required correction: Immediately, all electrical outlets accessible to children must have protective covers. Corrected at time of visit, the teacher put protective covers in the exposed outlets bringing the classroom into compliance. Submit a written statement of compliance by 12/12/2024. Correct by 12/05/2024
