Required correction: All outdoor equipment musr be sturdy, safe and free from hazards. Corrected at the time of inspection, staff removed the PVC piping from the playground. No response is necessary. Correct by 07/01/2026

A trash can on the preschool playground was broken with a large hole in the top of the plastic accessible to children and posing a potential safety hazard.
Required correction: Outdoor materials must be sturdy safe and free from hazards. Corrected at the time of the inspection. The Director placed duck tape on the broken plastic removing the hazard. No response is required. Correct by 10/11/2023

Upon the Specialist's arrival observed thumb tacks in the front lobby on bulletin boards in a common area that all children pass through.
Required correction: Thumb tacks must not be used in areas accessible to children under the age of three. Corrected at the time of the inspection. The Director removed the tacks from areas where children under the age of three. No response is required. Correct by 10/11/2023
