In Toddler 1, toys on the art shelf for use with playdough contained small parts that are a choke hazard for children under 3 years old.

Required correction: Immediately, all toys and toy parts accessible to children under 3 years old must not be a choke hazard.

In the hallway outside of Preschool 2, the corner of the rug was curled creating a tripping hazard. On the playground, there were rusting metal toys for the mud kitchen and broken plastic stacking blocks.

Jacklyn R. was observed working alone in the school-age classroom. Jacklyn has a CBI background clearance letter in their staff file, but did not have completed FBI clearance letter or TRAILS clearance letter results. Specialist observed documentation that both missing checks have been submitted. Jacklyn's DOH is 6/3/2024.

Required correction: All individuals requiring a background check must review Navigating the Background Check Investigation Unit's (BIU) Webpage powerpoint. Submit verification that staff member Jacklyn has reviewed the powerpoint within 24 hours. Submit documentation of FBI and TRAILS Clearance Letters for staff member Jacklyn with written response to Specialist. Correct immediately. *Corrected at the time of inspection, assistant director went into the school-age classroom with Jacklyn. Correct by 06/10/2024
