At the time of inspection the director was acting in both the director and infant program supervisor role. The other staff member who meets IPS qualifications arrived at 10AM, but leaves at 2PM, leaving the program without an qualified IPS sixty percent of the day.

Required correction: In the infant program there must be a qualified infant program supervisor present 60% of the hours of operation. Correct immediately on 6/4/2026. Provide statement of compliance by 7/4/2026. Correct by 06/04/2026

On the toddler playground two (2) classrooms ages 12-36 months were playing together with fifteen (15) children and 3 staff members.

Observed bells, small enough to fit in a choke tube in a sensory bottle with a loose lid in DPS 1 (12-36 mos.). Additionally, there were counting cubes, tanagrams, small bugs and a magnetic tree sorting game all with parts that fit in a choke tube observed in the PRS 1 room that currently had children under the age of three (3) enrolled accessible on low child size shelves.

Required correction: Materials accessible to children under the age of three (3) must not pose a choking hazard. Corrected at the time of visit by the director removing all the chokable items. NO RESPONSE REQUIRED. Correct by 06/04/2026
