In the bathroom between Room 136 and 137, diaper changing supplies were in unlocked plastic drawers and were accessible to children.

Required correction: Immediately, diaper changing supplies must be inaccessible to children at all times. Submit photo documentation and a written statement of compliance to the licensing specialist. Correct by 02/25/2026

Confirmed through staff statements, in rooms 122 and 123, the children are transitioning between classrooms during the preschool sessions and these transitions are not being tracked throughout the day.

Required correction: Immediately, attendance verification must be completed periodically throughout the day, including during transitions between classrooms. Submit documentation of the attendance tracking tool and a written statement of compliance to the licensing specialist. Correct by 02/25/2026

In room 122, a bottle of hand sanitizer labeled 'keep out of reach of children' was in a pouch on the side of the emergency backpack which was hanging low on the wall, and accessible to children.

Required correction: Immediately, all items labeled 'keep out of reach of children' must be inaccessible to children at all times. Corrected at time of visit, a classroom teacher moved the hand sanitizer to a high cabinet making it inaccessible to children, no response needed. Correct by 02/25/2026

On the preschool playground, the mulch within the use zone is compacted, and, as confirmed through staff statements, the mulch is not being raked regularly to maintain resiliency.
