In the Lyra classroom, a rug had a curled corner and edge creating a tripping hazard. On the Big Playground, plastic tubs and a dustpan in the mud kitchen were cracked and broken.

Required correction: Immediately, all indoor and outdoor equipment, materials, and furnishings must be sturdy, safe, and free of hazards. Corrected at time of visit, a cozy corner chair was moved onto the curled rug edge and the broken equipment on the playground was removed by the center director. No response needed.
