Outside on the preschool playground, a cracked yellow lid and a broken green shovel. In the toddler 2 classroom, the rug by the outside door was flipped up by the door. In the preschool 4 classroom, an unlocked cabinet had bubble wrap accessible to children. All were posing a potential hazard.

Required correction: All materials, furnishings must be safe and free of hazards. Corrected at the time of visit: Tammy removed the broken toys and moved the rug so it was flat, she also locked the cabinet.
