At the time of inspection, the specialist asked SM1 how many children were in attendance. The staff member initially responded that there were 9 children, then shortly after said 8. SM2 stated that there were 7 children. After reviewing the written classroom attendance, the specialist confirmed that 7 children were listed on the attendance roster.
Required correction: The time a child arrives and leaves the center each day must be recorded. Staff members must complete written attendance verification periodically throughout the day. Submit a written response of compliance to the specialist. Correct by 03/13/2026

Required correction: Exits must be arranged and maintained so as to provide free and unobdtructed egress from all parts of the building. *****Corrected at time of inspection, Director moved the shelf away from the door. No response is required. Correct by 04/10/2025

In the classroom observed wipes labeled "keep out of reach of children" on a low shelf in the childs bathroom accessible to children.
Required correction: Items labeled "keep out of reach of children" must be inaccessible to children. ****Corrected at the time of inspection, the Director removed the wipes and placed them on a locked cabinet, making the item inaccessible to children.

Reviewed 22 staff files and observed Ariana to be missing a current immunization training and identified that Ariana is primarily responsible for the collection, review and maintenance of child immunization records.
Required correction: Within 30 calendar days of employment and annually all staff responsible for the collection, review, and maintenance of the child immunization records must complete the immunization course. Submit a written response of compliance and training certificate to the specialist. Correct by 04/10/2025
