In the Infant A classroom, observed 2 cribs with sheets that were not properly fitted. No children were observed to be in the cribs at the time of the walk through.

Required correction: Mattresses for cribs and futons must have properly fitted, clean sheets. Corrected at time of visit-Staff adjusted sheets so that they were fitting properly on 1/16/26. No response required. Correct by 01/16/2026

Identified through staff statements that there was at least one incident in which 3 children were unsupervised in the bathroom resulting in children acting inappropriately.

Required correction: All children must be under the direct supervision at all times of a qualified adult who has been assigned the responsibility to supervise. Reveiwed name-to-face attendance rosters for 12/1/23, 12/8/23 and 12/18/23 and observed ECT Tamara to be recorded as working with Aide Dezirae on 12/8/23, from 9:47AM -10:10AM. Reviewed 10 staff files and observed Tamara's file to be missing verification of ECT qualifications.

Required correction: Obtain qualifications for all staff for the position in which they function, as required. Correct immediately and respond with verification of compliance by 1/9/24. Correct by 01/02/2024

During the initial walkthrough of classroom Pre K A, staff reported 13 children in attendance while there were 14 present. 13 children were noted on the attendance sheet. 1 child's attendance was not recorded upon arrival. During a follow up walk through, staff reported 14 children in the rooom. 12 children were present and 14 were listed on the attendance sheet. 2 children had not clocked out from the attendance sheet when they transitioned from the classroom.

Required correction: The time a child arrives and leaves must be recorded and staff must complete written attendance verification periofically throughout the day, including during transitions. Correct immediately and reply with plan for ongoing compliance by 1/9/24. Correct by 01/02/2024
