Reveiwed name-to-face attendance rosters for 12/1/23, 12/8/23 and 12/18/23 and observed ECT Tamara to be recorded as working with Aide Dezirae on 12/8/23, from 9:47AM -10:10AM. Reviewed 10 staff files and observed Tamara's file to be missing verification of ECT qualifications.

Required correction: Obtain qualifications for all staff for the position in which they function, as required. Correct immediately and respond with verification of compliance by 1/9/24. Correct by 01/02/2024

During the initial walkthrough of classroom Pre K A, staff reported 13 children in attendance while there were 14 present. 13 children were noted on the attendance sheet. 1 child's attendance was not recorded upon arrival. During a follow up walk through, staff reported 14 children in the rooom. 12 children were present and 14 were listed on the attendance sheet. 2 children had not clocked out from the attendance sheet when they transitioned from the classroom.

Required correction: The time a child arrives and leaves must be recorded and staff must complete written attendance verification periofically throughout the day, including during transitions. Correct immediately and reply with plan for ongoing compliance by 1/9/24. Correct by 01/02/2024
