Older complaints are released only on request. The department does not rank complaints by severity in either place.

At the time of the visit, an injury occured at the facility. The staff member followed the facilities, internal policy of reporting to administration. The administration failed to report an injury within the required licensing time frames.
Required correction: Immediately 5/19/2026, within 24 hours of an incident/injury to a child that requires medical attention by a health care professional must be submitted. Submit a written statement of compliance and documentation by 5/26/2026. Correct by 05/19/2026
