Observed a connecting game that contains small pieces on a shelf in the manipulative area accessible to children under the age of 3 years in the Fireflies classroom. This item can pose a choking hazard for children under the age of 3 years.
Required correction: Immediately 4/17/2026 toys, toy parts and all equipment and materials accessible to children under the age of 3 years must not be a choking hazard or able to be inhaled.
