Courtney and Kaitlin both indicated they have lived outside of the State of Colorado in the last five years. No supporting documentation was observed in the file at the time of inspection.

Required correction: Obtain Out of State clearance letter for staff members. Submit a written response of compliance to specialist along with documentation of Out of State clearance for each staff member. Provider must review Navigating the Background Check Investigation Unit's (BIU) Webpage powerpoint. Submit verification the provider reviewed the powerpoint within 24 hours. Notify Licensing Specialist in writing once the resident has completed the fingerprinting process. Correct by 05/02/2024
