Identified through provider statements that Clarie does not have TRAILS background clearance, and was working at the program as an assistant. At the time of inspection, Clarie was responsible for assisting with the care of 8 children ages 10 months -3 years of age.
Required correction: Trails requests must be made on the state-prescribed form, accompanied by the required fee within the following time frame. All individuals requiring a background check must review Navigating the Background Check Investigation Unit's (BIU) Webpage powerpoint. Submit verification that staff member Clarie has reviewed the PowerPoint within 24 hours. Submit documentation of Clarie for staff members with a written response to the Specialist. Correct by 01/08/2025
