At the time of visit, in Infants 2, both evacuation cribs had storage tubs underneath them, and one evacuation crib had a car seat stored in front of it, blocking it from the exit.
Required correction: Immediately, evacuation equipment must not block exit routes and nothing must be stored under any evacuation equipment at any time. Corrected at time of visit, the center director moved the storage tubs bringing the classroom into compliance. No response needed. Correct by 01/13/2025

In Room 8 and Preschool 6, there were several ripped books on the library shelf accessible to children. Throughout the center, shelves and equipment have chipped and peeling finish.

At the time of visit, a 15-month-old infant was placed in an unsafe sleep environment; Observed a child sleeping with a pacifier that had a cord attached to it creating a strangulation hazard.

Materials that could pose a suffocation or strangulation hazard are not permitted in cribs at any time for any reason.
