In the Lil Cubs classroom there was a small piece of chalk sitting on the art easel accessible to children under the age of 3 years old. The small piece of chalk was posing a possible choking hazard.

Required correction: Toys, toy parts, furnishings, equipment, and any materials accessible to children under than three (3) years of age must not be a choke hazard or able to be inhaled. Any area of the facility accessible to children less than three (3) years of age must be free of any choke or inhalation hazards. ***Corrected at the time of the visit a staff member disposed of the small piece of chalk on 11/13/2025. No response required.*** Correct by 11/13/2025

In the Skittles and Lil Cubs classroom there were cords longer than 18 inches long attached to air purifiers.

Outside on the playground there was a white broken shovel that had a sharp edge posing a stafey hazard.

Required correction: All outdoor play areas for children's activites must be checked dailt and kept safe and free pf hazards. ***Corrected at the time of the visit the director disposed of the broken while shovel on 12/16/2024. No response needed.*** Correct by 12/16/2024

In the little cubs there were 3 broken crayons and 4 glue sticks with small lids located in bins on the art shelf accessible to children under 3 years old posing a choking hazard.
