At the time of visit, and confirmed through provider statements, an adult over 18 years of age moved into the home over 5 days ago and has not completed the fingerprint background check.
Required correction: Immediately, all new residents 18 years or older must complete a fingerprint background check within 5 days of moving into the home. All individuals requiring a background check must review Navigating the Background Check Investigation Unit's (BIU) Webpage powerpoint. Submit verification that resident Eric A has reviewed the powerpoint within 24 hours. Submit documentation of a completed fingerprint background check appointment and a written statement of compliance by 12/12/2024. Correct by 12/05/2024

At the time of visit, there was a pair of adult scissors on the teacher table, and staples loose in the wall, both accessible to children.
Required correction: Immediately all hazardous items must be inaccessible to all children at all times. Corrected at time of visit, the provider removed the loose staples and moved the scissors to a high shelf making them inaccessible to children. No response needed. Correct by 12/05/2024

At the time of visit, the knobs on the gas stove lacked a safety guard and were accessible to children.
Required correction: Immediately, all cooking stoves with controls accessible to children must have a safety guard. Corrected at time of visit, the provider put up gates to block off the kitchen making the stove inaccessible to children. No response needed. Correct by 12/05/2024
