Substitute was being used to provide supervision of children as the primary provider was absent. Identified through staff statement parents/guardians of children in care were not notified of substitute supervision. Primary provider was absent for approximately thirty minutes and returned to the home while the visit was still in progress.
Required correction: Parents/Guardians must be notified each time a substitute is used to provide supervision of children in the absence of the primary provider, applicant 2, or equally qualified provider. Correct immediately. Submit written verification to specialist. Correct by 04/27/2026
