Required correction: All indoor and outdoor equipment, materials, and furnishing including the black playground tiles must be sturdy, safe, and free of hazard. Correct immediately and respond with verification of compliance by 12/9/2025. Correct by 12/02/2025

Outside on the plaground the large green swing had a broken link on the chain holding the swing up.

Required correction: Ensure that all outdoor equipment like the large green swing must be sturdy,safe, and free of hazard. ***Correcet at the time of the visit the staff put caution tape around the swing to ensure the swing can not be used.*** Respond immediately with verification of compliance by 2/14/2024. Correct by 02/07/2024
