Reviewed 10 staff members files. Observed 1 staff members( Judy J. ) files missing department approved certificate for mandated child abuse reporter.

Required correction: Correct immediately. All staff members must be trained using a department approved training on how to report and when to report suspected or known child abuse or neglect. Corrected at the time of the visit, director completed the training during the visit. Correct by 07/11/2024
