Required correction: Toys, toy parts, furnishings, equipment, and materials made of brittle, easily breakable plastic or glass are not permitted for children less than five. Corrected at the time of the inspection. The glass was removed from both classrooms. No response required. Correct by 04/14/2025

Reviewed 29 staff records and observed two staff member's Teja G. (DOH 3.5.2024), Ruby. S (DOH3.11.2024) and Tykia Q. (DOH 5.8.2023) to be missing documentation of the required out of state background clearance letter. Identified that both staff members lived out of the state of Colorado in the past five years.

Reviewed 29 staff records and observed one staff members Maria A. (DOH 9.2018) to have completed a CPR and First Aid course through an unapproved vendor and the staff member to be missing the required CPR and First Aid module.
Required correction: All employees caring for children, not required by rule to be certified in First Aid and CPR, must complete the Department-approved Introduction to First Aid and CPR module. Correct by 5.14.2025, and submit a written statement of compliance to the Specialist. Correct by 05/14/2025

Shortlist asked for that file under the Colorado Open Records Act and renews it yearly; it is what this page shows for complaints older than the department’s own window.
