A bin of small shape pieces, accessible to children, in an unlocked lower cupboard, in classroom 142. The bin contained a number of smaller plastic shapes that pose a potential choking hazard to children under age 3. It was identified through staff statements that children under age 3 use the classroom.

Required correction: Any materials accessible to children under age 3 must not pose a choking hazard. Correct by 5/19/26. Staff moved the plastic shapes to an upper locked cupboard, making them inaccessible to children. Corrected at the time of inspection. No response necessary. Correct by 05/19/2026
