At the time of the visit, in the Infant room a swaddling sleep sack was observed in one of the cribs, (without infant) confirmed through a staff member's statement that this swaddling sleep sack is being used for one of the children without a health care provider care plan.
Required correction: Correct immediately. Swaddling of infants must only be allowed with the help care providers signed plan.
