Required correction: Every exit must be clearly visible, or the route to reach it must be conspicuously indicated. Each path of escape must be clearly marked. At the time of the visit, observed Ms. Jaina, Ms. Thalia and Ms. Shira to be the assigned staff member to the Caterpillars classroom. Ms. Ash, Ms. Jackie and Ms. Beau to be the assigned staff members to the Dragonflies classroom. Ms. Katelyn, Ms. Lily and Porter to be the assigned staff members to the Love Bugs classrom. Ms. Brigette and Ms. Lindsay to be the assigned staff members to the Fireflies classroom

At the time of the visit, observed 8 children in the Dragonflies group of children in the playground, the staff member reported that they had 10 children. The staff members with the Fireflies group of children and the Pandas group of children weren't sure about the number of children they are caring for in the playground. Also observed 10 children in the Tigers group of children in the playground, the staff member reported that they had 11 children.

Required correction: Immediately (7/17/2025) the playground area must be in good repair, of solid and safe construction, free of rough edges, protruding bolts. Corrected at the time of the visit, a staff member fixed the nails and made the sharp edges of the wires inaccessible to children and removed the loose wire.
