Classroom 131 was playing outside and did not complete a written name to face attendance verification when coming back inside from the outside play area. In classroom 129 there were 15 children present and only 14 children signed in on the attendance sheet.

Required correction: The time a child arrives and leaves the center each day must be recorded. Staff members must complete written attendance verification periodically throughout the day, including during transitions. Correct immediately and respond with verification of compliance by 12/18/2025. Correct by 12/11/2025

Confirmed through a staff memeber's statement another staff member was in the cafeteria with 5 child having snack. After a staff file review it was confirmed that staff member #5 is not a qualified early childhood teacher.
