At the time of the visit classroom 122 and classroom 123 were all outside playing on the playground together. Classroom 122 had 17 children ages 4 to 5 years old and Classroom 123 had 18 children ages 4 to 5 years old. Outside on the playground the two classrooms were over the max group size by 11 children ages 4 to 5 years old.

Required correction: The facility must follow the max group size. ***Corrected at the time of the visit classroom 123 with 18 children went back inside on 5/7/2026. Respond with a plan for ongoing compliance by 5/14/2026. Correct by 05/07/2026

In classroom 123 there were 20 children present and only 15 children were signed in on the attendance sheet. In classroom 122 there were 19 children present and only 17 children signed in. Also it was idenitfied through staff members' statements they are recording the name to face that are completed at each transition.

Required correction: The time a child arrives and leaves the center each day must be recorded. Staff members must complete written attendance verification periodically throughout the day, including during transitions. Correct immedialty and respond with a plan of ongoing compliance by 9/27/2025. Correct by 08/27/2025
