Specialist asked staff member in Room 120 how many children they had, staff member stated 18, double checked daily attendance then said 17, then specialist counted 16 children and staff member confirmed that 16 children were present. Specialist identified that no periodic attendance verification had been taken so far during the day.

Required correction: Staff members must complete written attendance verification periodically throughout the day. Correct by 09/19/2024

Child proof door handle lock installed on the front door in room 120 and a baby gate installed in the back door to the outside in room 119 both preventing free and unobstructed egress from the exit doors.

2.217.A.15 · Correction due September 19, 2024

29 children ages 2.5-6 years all playing on the preschool playground from classrooms 102 and 103. Specialist did not observe Rooms 102 and 103 operating independent of each other and children were mixed together across the playground. Specialist also observed 22 children ages 2.5-6 years in Room 119 and 22 children ages 2.5-6 years in room 118 both operating over group size and children in both rooms were all together and operating as one group with no separation of defined groups.

Required correction: Maintain proper group size at all times. Submit plan of action for outdoor schedule with written response to specialist. Correct by 09/19/2024
