In classroom 127 there was one child missing a time the child arrived on the attendance sheet. In classroom 128 the teacher did not complete a name to face attendance verification sheet. A group of children went for a health screening outside the classroom.
Required correction: The time a child arrives and leaves the center each day must be recorded. Staff members must complete written attendance verification periodically throughout the day, including during transitions. Outside on the playground there was a gap on the green climber attached to the play structure posing a possible strangulation hazard. Also there was a small pea size hole close to the top of the green climber attached to the play structure. There is also a gap between the top of the green side by side slides posing a possible strangulation hazard. Outside through out the entire playground that was a lot of debris on the ground. Outside on the grassy area there was a large area of uneven ground and a hole posing a possible tripping hazard.
Required correction: All outdoor play areas used for children's activities must be checked daily and kept safe and free of hazards. Correct immediately and respond with verification of compliance by 11/12/2024. Correct by 11/05/2024

In the sandbox there was matting stick up out of the sand posing a tripping hazard.
