At the time of the visit licensing specialist walked into classroom 103 and more than half of the children still wide awake on their mats. There were 19 preschool age children to 1 staff member.

Required correction: At nap time, the child to staff ratio may be doubled for children two and one half (2 ¥%) years of age and older in preschool classrooms when the following conditions have been met:

In classroom 108 licensing specialist reviewed the name to face transition sheet and there were only 14 children marked on the name to face at 12:20. There were 15 children present at the time of the visit. In classroom 120 licensing specialist completed a review of the name to face sheet. There was a transition time listed at 12:45PM but the two staff members did not completed it on the sheet until after the children were back inside and laying down for a nap.

Required correction: The time a child arrives and leaves the center each day must be recorded. Staff members must complete written attendance verification periodically throughout the day, including during transitions. ***Corrected at the time of the visit the staff members marked all children in on the name to face tranistions sheets on 4/20/2026.*** Respond with a plan for ongoing compliance by 4/27/2026. Correct by 04/20/2026

At the time of the visit licensing specialist walked into classroom 123, and it was very dark and it was hard to see the children lay on their cots.

Required correction: In rooms used for napping, the lighting must be dim at nap time to promote an atmosphere conducive to sleep but must be bright enough for supervision of children. Correct immediately and respond with a plan for ongoing compliance by 4/27/2026. Correct by 04/20/2026

Licensing Specialist walked into classroom 127 and staff member 1 was inside the classroom with 1 child and the Para was outside on the playground with 13 children ages 3 to 5 years old. Outside on the playground the teacher was over by 3 children. Confirmed through staff members statement the para was outside alone with 13 children for approximately 5 minutes.

Required correction: Ensure that all groups of children including classroom 127 must maintain staff to child ratio. ***Corrected at the time of the visit the teacher and the one child went outside to the playground.*** Respond immediately with an ongoing plan for compliance by 5/13/2024.
