There was a tube of sunscreen labeled "keep out of reach of children" sitting in a bag near the top of the stairs leading to the basement that was accessible to children. The provider's purse was sitting on a chair in the kitchen accessible to children posing a possible safety hazard.

Required correction: All hazardous items including tubes of sunscreen and the provider's purse must be inaccessible to children. ***Corrected at the time of the visit the provider places the sunscreen and the provider's purse up high and inaccessible to children on 10/15/2025. No response required.*** Correct by 10/15/2025

There two children under 3 were playing with barbies and the shoes and crown were posing a choking hazard.

Required correction: Ensure that all items including small toy parts like barbie shoes and crown must not be accessible to childen under 3. ***Corrected at the time of the visit the provider removed the barbie shoes and crown making them inaccessible to children. No need for written statement of of compliance.*** Correct by 12/12/2023

In the hallway leading to the licensed basement there was a small treadmill.

Required correction: Ensure all exercise equipment must be made inaccessible to children. ***Corrected at the time of the visit the provider placed the treadmill into the locked garage making it inaccessible to children. No need for written statement of compliance.*** Correct by 12/12/2023
