Reviewed 19 staff files and identified 17 staff members (SM) lacking verification of TRAILS clearance letter maintained in staff file. (SM#1-3, 6-7,10-11,13,15, 17-24).
Required correction: All individuals requiring a background check must review Navigating the Background Check Investigation Unit's (BIU) Webpage powerpoint. Submit verification that staff members have reviewed the powerpoint within 24 hours. Violation partly corrected on 10/8/25, background check clearances for those staff listed other than SM10 and SM23 were received and verified by Licensing Specialist on 10/8/25. Submit documentation of TRAILS background check for staff members SM10 & SM23 with written response to the Specialist. Correct by 10/07/2025

Area in back corner of Toddler playground with several wooden boards with splintering wood around garden beds and area of ripped/torn weed matting under the nearby tree, posing potential safety hazards.
Required correction: Immediately ensure all outdoor play areas used for children’s activities must be checked daily and kept safe and free from hazardous materials or debris by removal of debris, dilapidated structures, and broken or worn play equipment. The staff must identify hazardous, high-risk areas; those areas must be made inaccessible to children to reduce the possibility of injuries and accidents. Violation partly corrected at the time of visit. Staff were made aware of high-risk area and placed cones around area temporarily. Document complete correction in written response. Correct by 10/07/2025
The Department of Early Childhood keeps about three years of complaints on a program’s public page. Anything older is released only on request, as a single file covering the whole state.
